ios: accept bounded outage states

- Allow a clean pre-commit failure with no possible remote effect.
- Allow one explicit possible orphan after an indeterminate upload.
- Reject every other unavailable-media presentation state.
- Preserve the relaunch and reconnect retry requirements.
