ios: preserve indeterminate outage evidence

- Treat an unreachable upload as an indeterminate remote effect.
- Assert the device exposes one possible orphan for the failed media.
- Keep the unverified draft available across application restart.
- Preserve the follow-up reconnect and retry qualification path.
