tests: rename fallback-shaped coverage

- describe bootstrap presence coverage as the application handler path
- describe subscriber wrapper coverage as direct and hook paths
- keep runtime behavior and assertions unchanged
- validate bootstrap presence and subscriber io filters through extbuild
