docs: document discovery relay provenance

- describe per-relay live fetch behavior and grouped event source relay output
- document relay_states and relay_summary in discovery diff and refresh responses
- explain cross-relay divergence versus stale relay history for conflicted live state
- keep the discovery guide aligned with the current command surface
