docs: document discovery sync lifecycle

- document inspect-live-nip89 diff-live-nip89 and refresh-nip89 command surfaces
- describe missing matched and drifted live states and the default no-op refresh behavior
- keep the guide aligned with the implemented runtime audit visibility and serving boundary
- keep discovery operations documented from the repo root
